She's brought Ichigo (and Menoly) back to life...

Melony might not have been dead. It's possible she was still somewhat alive. Though she did seem pretty dead.

Ichigo just came back on his own somehow. I assumed it was because they were in Hueco Mundo and that's pretty much an afterlife area.

Oh no Ichigo was very dead, and she revived him, Inoue can undo pretty much anything, wounds, death, probably memories too if she wanted

I had actually forgotten about her, dammit why is Bleach so full of plot devices

It's actually supposed to be more powerful than that. She's able to "deny events," which I take as being able to re-write the very fabric of existence to erase anything that happened (such as a fatal wound). Basically, she's a god. If Aizen had that power, he'd have already won.

I said Melony might not have been dead. Not Ichigo.

Ichigo had a hole in his body. Inoue never used her ability on him though. She just kept yelling. Ichigo's hollow took over.

Oh that time, I was thinking of the first time Ulquiorra killed him, you know when Grimmjow brought Inoue to revive him

Ichigo wasn't dead that time either. Grimmjow even mentioned that Ulquiorra had a habit of stabbing people in the center of the chest instead of the heart.

Which is just as lethal though, punching holes anywhere in someones torso is pretty lethal

Also I think he was dead, at least he looked and seemed very dead

We're talking about a series where just about everyone has had a limb torn off.

In any manga you can pretty much be assured that being stabbed through the chest isn't lethal unless it hits their heart.

That hole was definitely through the mediastatium. Important things it hit:
1. Trachea
2. Superior vena cava
3. Spinal column
4. Solar plexus (not an important organ in and of itself, but it helps keep the ribs from collapsing inward)
5. Esophagus
It also probably hit a bunch of less major arteries and veins, which should make him bleed to death fairly quickly. That being said, so should having a limb torn off.

Heart
Lungs
stomach

I know Bleach and a lot of other animes are ridiculous when it comes to wounds, but that's enough to bring down kenpachi, I still say he was dead

A hole that size took a chunk out of both lungs and probably hit the right atrium. Not instantly fatal, but I'd give him less than five minutes before he either bled to death or died from lack of oxygen (using your anime anatomy). Though I'd think anime does have tracheas, because people can get choked, and that's definitely completely gone from that hit.

Looking back on where exactly he was pierced, he most definitely got hit through the heart (your heart's not on the left side of your body, it's in the center, but sticking out to the left)

Hm...I think I can see some organs besides the stomach, lungs, and heart in that picture...
Also, the brain counts as an organ.

Um...assuming Arrancar have a similar physiology to humans (they seem to bleed just like them), the heart is located in the mediastinum. Media as in middle. On humans, the base of the heart is pretty much dead center, but the apex skews to the left.
